Location: Floyd Central High School Sports Complex Soccer Field

Parking: Off Edwardsville Galena Rd. Soccer Field is behind Stadium.

• Participants must arrive 30 minutes early to Check-In (12:30pm). 
• Participants should be warmed up and ready for the tryout to begin at 1:00pm.
• Players Must Register Online.
• Registration Cost is $50.00
• Registration fees are non-refundable.

**SOCCER TRYOUT LIABILITY WAIVER** I, the Applicant Have read the following waiver and agree to the terms contained therein, by checking the Applicant Waiver Consent Option on the following page, I agree to be bound by the terms of the waiver below.

I, (hereinafter referred to as the "Applicant"), in consideration of my and/or my child's or ward's participation in the Soccer Tryouts, do hereby agree to release and hold harmless Southern Indiana Guardians FC LLC (SIGFC) and Floyd Central High School, its officers, coaches, volunteers, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me or my child or ward, while participating in the Soccer Tryouts, or while on the premises where the event is being conducted.

I am fully aware of the risks and hazards connected with participating in this event, and I elect to voluntarily participate, knowing that the associated physical activity may be hazardous to me, my child or ward, and our property. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or my child or ward as a result of participating in this event.

I further agree to indemnify and hold harmless Southern Indiana Guardians FC LLC (SIGFC) and Floyd Central High School, its officers, coaches, volunteers, and representatives from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my or my child's or ward's participation in the Soccer Tryouts, whether caused by the negligence of the Soccer Club or otherwise.

It is my express intent that this Waiver and Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a release, waiver, discharge, and covenant not to sue the Soccer Club.

I, the Applicant, hereby grant Southern Indiana Guardians FC LLC permission to use my, or my child's or ward's likeness in advertising and commercial purposes.

I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Indiana.
